I'm just a study coordinator, does the Policy on Conflicts of Interest in Research and Sponsored Programs apply to me?  

This Policy applies to all Medical Center personnel who design, conduct or report on research or other sponsored projects at the Medical Center.  All investigators and other key personnel (including study coordinators) on sponsored project submissions need to know this Policy and submit a disclosure form on a per project basis.

I received an Email from CTMailServer@mediregs.com telling me I need to complete an Annual Disclosure  Form.  Is this junk Email that I should ignore?

No, do not ignore this Email. You will be notified via Email when it is time to complete the Annual Disclosure Form and receive periodic reminders until your form is complete.  PLEASE DO NOT DISREGARD and/or DELETE any Emails from Keisha Lightbourne, or Monique Phillips and/or identifying MediRegs (e.g. CTMailServer@mediregs.com) the company that designed the Annual Disclosure Form.     

I already submitted my Annual Disclosure Form.  Do I have to submit another disclosure form for my research study?

You must complete and submit an Annual Disclosure Form and an Investigator Financial Interest Disclosure Form in connection with each research or other sponsored project in which you participate.  The Investigator Financial Interest Disclosure Form is required:

• When you first submit a project to the Medical Center (and before submission to external sponsors);
• Every year you continue the project; and
• Before acquiring any new interest in any entity that might affect, or be affected, by the research or sponsored project

Why do I have to submit both an Annual Disclosure Form and an Investigator Financial Interest Disclosure Form?

The Annual Disclosure Form asks for disclosure of all of your financial interests.  The Investigator Financial Interest Disclosure Form asks you to specify only those financial interests you feel affect or may appear to affect the specific research project.

What am I required to disclose in the Annual Disclosure Form?

You will be required to disclose each of the following interests for you and your Immediate Family:

• all entities in which you have a Financial Interest that
  currently competes or does business with the Medical Center

• all entities in which you have a Financial Interest that

  may compete or do business with the Medical Center
