Annual Disclosure

 

The Policy on Conflicts of Interest in Business Affairs requires that certain NYULMC Members submit an annual disclosure form to the Conflicts Management Unit.  Completion of this disclosure form will be requested annually at different times depending on your role at the Medical Center.  For example, full and part- time, paid faculty will be required to complete their disclosures every spring in order to align with requirements imposed by the Main Campus.


Accessing the Form

The Annual Disclosure Form is housed in a web-based, password accessible, electronic survey tool.  As such, it is accessible 24 hours a day from any internet accessible computer.  Once a form is completed, it can be reviewed and updated throughout the year, which will make the annual disclosure process a much smoother one for you.

PLEASE NOTE: You will be notified via Email when it is time to complete the Annual Disclosure Form and receive periodic reminders until your form is complete.  PLEASE DO NOT DISREGARD and/or DELETE any Emails from Keisha Lightbourne, or Monique Phillips and/or identifying MediRegs (e.g. CTMailServer@mediregs.com) the company that designed the Annual Disclosure Form.
