Who Needs to Complete the Investigator Financial Interest Disclosure Form 

 

All Investigators must submit an Investigator Financial Interest Disclosure Form.

When Should You Submit Your Investigator Financial Interest Disclosure Form 

 

  1. Upon submission of each proposal of a Sponsored Project to the IRB, IACUC or Sponsored Research Office, even if the Investigator does not have a Financial Interest to disclose; 
  2. At each annual continuation of the Sponsored Project thereafter; and
  3. Prior to obtaining any new Financial Interest during an ongoing Sponsored Project.

NOTE REGARDING SAVING YOUR FORM IN ADOBE:  You can save the completed form only if you have the Adobe Professional version.  If you do not have this version of Adobe, please contact IT to install since NYU has a licensing agreement for this software.  

NOTE REGARDING SAVING YOUR FORM IN WORD:The form has also been made available in MicroSoft Word to enable you to save your data. However, please note that using this version may affect formatting.  Otherwise, you can simply print out a hard copy of the completed form to maintain a record of your data. 

We are currently working on a solution so all users can save the document for future use.  In the meantime, you can either save the document in MicroSoft Word or print out the completed form as an alternative if you do not have Adobe Professional.

*What to do if You Answer Yes

If you answer "Yes" to any of the questions on your Investigator Financial Interest Disclosure Form, You Must Complete the SUPPLEMENTAL Investigator Financial Interest Disclosure Form.

 

 

Confidentiality. All disclosures will be kept confidential and disclosed only on a need-to-know basis as required to perform the review and evaluation required by this Policy and the Medical Center's Policies on Conflicts of Interest, Commitment and Consulting.
